Sam Forzley, O.D.

Dr. Forzley is a 1986 graduate of the Illinois College of Optometry and Illinois Eye Institute. Certified by both the National Board and Illinois Board of Examiners, Dr. Forzley’s postgraduate training included an internship at Fort Sheridan Army Base and the Chicago Lighthouse for the Blind. Dr. Forzley began his career with LakeShore Eye Physicians and Surgeons as a contact lens specialist. He then joined Wheaton Eye Clinic and was a staff doctor for 20 years, with an emphasis in Neuro-Ophthalmology and Glaucoma. Dr. Forzley started Forzley Eye Clinic in 1991 providing the people of Lemont and surrounding communities with expert eye care – specializing in the treatment of eye diseases and conditions, as well as contact lens fitting. Dr. Forzley is a member of the American Optometric Association and the Illinois Optometric Association, currently serving as their secretary and treasurer. Dr. Forzley is a member of the Lemont Lions Club and has been privileged to provide free eye examinations those in need for 28 years. 2018 he was honored with the Lions Club Melvin Jones Humanitarian Award.

Dr. Forzley and his wife Missy have been married for 36 years and have four children, Sarah, Nick, Anne, and Elizabeth, and three grandchildren, Grace, Jack, and Hank. His hobbies include all outdoor activities, traveling, and sports (especially college football). As a well-known “Parrothead,” he enjoys attending Jimmy Buffett concerts and has lost count after attending over 50 shows.

Most of all, he sincerely loves spending time with his family and friends.
